The Minnesota Immunization Information Connection (MIIC) is a statewide immunization information system that stores electronic immunization records. MIIC combines the vaccines a person has received into a single record, even if they were given by different health care providers in the state. MIIC helps Minnesotans get the right vaccines at the right time.

The information in MIIC is classified as private and only those authorized by law (Minnesota Statutes, Section 144.3351) can access it. Complete the following form to request your or your child's MIIC immunization record. This form is for public requests only. Providers should email the MIIC Help Desk directly at health.miichelp@state.mn.us for access to patient records.

Once you have submitted your request, MDH will send the requested record within 21 business days. If you have any questions about MIIC record requests, please email Health.immrecords@state.mn.us.

Though most Minnesota health care providers participate in MIIC, some do not regularly submit immunization information. Also, because MIIC was started in 2002, vaccines given before that may not appear in your MIIC record. See tips for finding old immunization records at Find My Immunization Record.
